You are advised to contact the appropriate State Vital records for any fees required to re-register the birth certificate as well as receive a certified copy. For adoptees born in the state of Washington, you can obtain information at the Department of Health website,
www.doh.wa.gov, or call them at (360) 236-4300. For adoptees born outside the state of Washington, you can obtain information at
http://www.cdc.gov/nchs/w2w.htm and click on individual State.
On the day when the adoption is finalized, you are to provide the Clerk’s Office with two copies of the Application for Adoption Registration form (to obtain a new birth certificate), the correct amount of money and the Adoption Data Card.
If you want to obtain a certified copy of your Decree of Adoption, please have cash as we do not accept personal checks or debit/credit cards. The fee for a certified copy is $5.00 for the first page and $1.00 for each page thereafter, for each document. Copies are only available on the day of the adoption and the record is then sealed to all parties.
