Chronic Disease Prevention


​Six in ten Americans live with at least one chronic disease, like heart disease and strokecancer, or diabetes. These and other chronic diseases are the leading causes of death and disability in America, and they are also a leading driver of health care costs. Island County Public Health helps people and communities prevent chronic diseases - promoting health and wellness for all.
Current Island County Activities
  • Island County WIC Program - Supporting access to healthy foods and making connections to health services.

  • Community Outreach Program - Island County community outreach workers provide resources for optimal health, discuss steps for home health, and make connections with locally available resources. We support residents across the lifespan, providing resources and linkages to support conditions like childhood asthma, diabetes, heart health, long COVID, and hypertension. Services are available in English & Spanish. Outreach workers also plan health fairs and workshops across Island County to improve access to services and share health resources. Workshops are offered at convenient community locations in English & Spanish, and are based on the Eat Healthy, Be Active curriculum from Health.gov.   • Community Health Planning - We engage in comprehensive community health assessment and planning to understand and address a variety of factors related to chronic disease outcomes, working in collaboration with healthcare providers and community organizations. Comprehensive health planning helps set goals for interventions and track progress.

WA State Breast, Cervical and Colon Health Program

This program provides free breast and cervical cancer screening to eligible people in Washington State. Eligibility for the BCCHP is primarily based on health insurance status, income, and age. Find out if you may be eligible.
